Transient Students
Students can submit their application, documentation, and any other supporting materials to the Center for Disability Services/SNAP to process via SecureShare. Students should send us their materials in SecureShare with their College of Charleston email account
- CDS-SNAP application form pdf format or Microsoft Word (.doc) format
- If completing page 3 of the application (Authorization and Consent to Release SNAP Information) please review the details below about the submission of that form.
- A letter from your home institution which includes:
- a statement that the school has received documentation of a disability and has approved you for services
- a list of the services which the home institution provides and you routinely use
- the signature of the home institution's Director of Disability Services (or their designee) and the name and phone number of that contact person
- A copy of the evaluation which documents the diagnosis(es) and includes data supporting the accommodations that you are requesting
Once your complete application packet is submitted, an administrator evaluates the application and supporting documentation, and makes a recommendation of approval or denial. A letter of approval or denial will be sent to your College of Charleston email address approximately five to ten business days of receipt of your materials.
If your request for accommodations is approved, the email you receive will include a CDS/SNAP Acknowledgement Form and a Student's Guide to SNAP Services. Please refer to the approval letter to learn about the remaining steps required to receive your Professor Notification Letter (PNL).
If your request for accommodations is denied, you may appeal to the Director of the Center for Disability Services.
