Deaf or Hard of Hearing (HOH)
The following professionals are considered qualified to evaluate and diagnose a hearing impairment:
- Otorhinolaryngologists (ENT)
- Otologists
- Audiologists
Documentation from a current treating provider (as noted above) should be on letterhead, be signed and dated, and should include:
- Discussion of type, degree and configuration of hearing loss, including frequency and intensity
- Description of any audiological technologies currently used (e.g., hearing aids, assistive listening devices, cochlear implant, sign language interpreters, real-time captioning (please include specific brand names, model #, etc. if applicable)
In lieu of a report, your treatment provider can complete the disability verification form: pdf format or Microsoft Word (.doc) format
