Attention Deficit / Hyperactivity Disorder (AD/HD)
The following professionals are considered qualified to evaluate and diagnose AD/HD:
- Licensed psychologist or neuropsychologist
- Licensed school psychologist
- Psychiatrist
- Physician (M.D.) specializing in the diagnosis and treatment of attention deficits
Documentation from a current treating provider (as noted above) should be on letterhead, be signed and dated, and should include:
- Information regarding a sufficient number of symptoms of Inattention and/or Hyperactivity/Impulsivity that have been persistent and maladaptive
- Information regarding the childhood onset of symptoms and associated impairment
- Information regarding executive functioning including memory, concentration, attention and discussion of strengths and impact of condition(s) in the postsecondary environment.
